Key Performance Indicators (KPIs)

Definition: Percentage of pilot participants who move into stable, permanent housing within three months of exiting the Grace Place program.
Pilot target: 80%
Measurement method: Confirmed housing outcome documented in exit survey and verified by lease agreement, landlord confirmation, or housing placement record.
Data source: Participant exit survey, case manager records, and housing documentation (lease/placement confirmation).
Reporting cadence: Quarterly and in the 12-month pilot evaluation.

Definition: Percentage of participants who enroll in or actively engage with employment, job-training, education, or vocational programs during their program participation. Engagement counts as enrollment plus at least one documented participation activity (class, interview, employer contact, or coaching session) during the pilot.
Pilot target: 70%
Measurement method: Enrollment and participation tracked via case manager logs and participant self-report; verified by program enrollment records, employer contact, or coaching attendance logs.
Data source: Case management case notes, coaching attendance sheets, employer/training confirmations.
Reporting cadence: Quarterly and in the 12-month pilot evaluation.

Definition: Percentage of participants who maintain recommended prenatal and/or postpartum healthcare visits during program participation (e.g., attending scheduled appointments). For postpartum participants, compliance includes at least one postpartum visit within 6 weeks.
Pilot target: 100%

Measurement method: Appointment attendance tracked via participant-consented health records, case manager confirmation, or provider verification. Missed appointments and reasons will be logged.
Data source: Participant health authorization forms, provider confirmations, and case manager records.
Reporting cadence: Quarterly and in the 12-month pilot evaluation.

Definition: Average amount of participant-held savings (in escrow or verified personal account) at program exit. This measures financial resilience and readiness for housing stability.
Pilot target: Average savings at exit ≥ $1,000
Measurement method: Summation of verified savings balances per participant at exit (savings escrow account statements, bank verification, or documented program-managed savings). Report the mean and median.
Data source: Savings escrow/account statements or case manager verification forms.
Reporting cadence: Quarterly snapshot and final pilot report.
